Four steps to submit

Download the PDF form using the button above. You can print it, or fill it in with any PDF editor.
STEP 02
Fill in & sign
Complete all five sections clearly. You and both guarantors sign and date section 5.
STEP 03
Gather documents
Scan or photograph your photo ID, driver’s license and portfolio summary so every detail is readable.

What goes in each section
Please write clearly and complete every required field. Incomplete forms will be returned.
Section 1
Applicant Information
- Full Name exactly as it appears on your government-issued photo ID.
- Mobile Number with country code, e.g. +91 98765 43210.
- Email Address you can be reached on — ideally the one registered with your MCL account.
- Residential Address in full, including city, state/province and postal code.
- MCL ID — your member ID, shown in your dashboard after you log in.
Section 2
Referrer / Sponsor Details
- Two guarantors are required. Both must be existing MCL members.
- For each guarantor give their MCL ID, Full Name and Email Address.
- Let your guarantors know before you apply — we may contact them to confirm.
Section 3
Loan Details
- Requested Loan Amount — write the figure and the currency (e.g. 500 USDT).
- Purpose of Loan — a short, clear description of what the funds are for.
- Trading Portfolio Details — the platforms or wallets you trade on and an approximate current value.
Section 4
Required Attachments
- Tick each box for the documents you are attaching.
- Government-issued photo ID and Driver’s License are mandatory.
- Add a Trading Portfolio Summary (statement or screenshot) and any other supporting documents.
Section 5
Signatures
- You and both guarantors must sign and date the form.
- Unsigned or partly signed forms cannot be processed.

- Both guarantors are MCL members and have signed
- Photo ID and driver’s license are attached and readable
- Attachments are PDF, JPG or PNG
- The subject line includes your full name and MCL ID
